GENERAL OVERVIEW:

Receives intake calls from individuals in the community in need of information and referrals to financial assistance/education/and community healthcare services.   Assesses client needs and provides appropriate referrals. Keeps current and is a respected source of knowledge for co-workers, network providers, social work, volunteers, etc.  Provides support in coordination and execution of any community outreach program initiatives.  Strong knowledge of Microsoft Word and Excel required.  This individual should be a good team player and have a genuine interest in serving the community. 

ESSENTIAL RESPONSIBILITIES:

  • Receives intake calls from the local community. 
  • Assesses client needs and develops appropriate referrals. 
  • Keeps current and is a respected source of knowledge for co-workers, network providers, social work, volunteers, etc. 
  • Areas of specialized expertise develop naturally as an outcome of intensive research to update knowledge base. 
  • Assists in the development and implementation of a variety of programs and services to enhance department assistance to seniors and others in the community.
  • Demonstrates accountability for the department functioning and service to clients and each other.  Operates in a team framework (scheduling, problem resolution, communication, etc.).  
  • Contributes to staff needs and complements other staff assisting them in their journey to continuous improvement. 
  • Demonstrates an awareness of our interdependency and need to work collaboratively to be successful. 
  • Assists with coordination, execution and evaluation of community education programs.
  • Attendance at external community meetings as assigned.

QUALIFICATIONS:

Minimum

  • Bachelor’s Degree in a human services or similarly related field
  • 3-5 years’ experience in a human services field.  Computer competent.  Ability to navigate and use the Internet and other I & R databases.
