GENERAL OVERVIEW

Thoroughly reviews inpatient medical records as required by the Pennsylvania Health Care Cost Containment Council, the Joint Commission Core Measures program, the Centers for Medicare and Medicaid Services (CMS) and facility/system-wide performance improvement initiatives to identify and abstract data into computerized databases.

ESSENTIAL RESPONSIBILITIES

  • Completes an abstract for each medical record as required by the Atlas system and as required for special external reporting requirements such as JC Core Measures, PHC4, CMS and the organization's quality improvement program, etc. Pulling and accurate wanding of medical records, completing requests for missing documents, filing documents on chart, completing daily work statistics and completion of terminal digit assignments. (90%)
  • Completes required information system functions, such as data imports and exports. (10%)
  • Performs system functions and report running as needed. (5%)
  • Performs other duties as assigned or required.

QUALIFICATIONS

Minimum

  • Minimum 4 years experience performing medical record review and data abstraction.
  • Strong computer skills.

Preferred

  • Associate's Degree preferred
  • Successful completion of college level courses in medical terminology and anatomy & physiology strongly preferred
  • RHIT/RHIA preferred
  • Familiarity with pharmaceutical terms preferred
