GENERAL OVERVIEW

The Oncology Nurse Navigator guides patients through the WPAHS oncology care programs, ensuring coordination of services to the patient and serving as clinical resource to the patient, family and care providers; addresses community needs for cancer screening services, cancer education and cancer wellness/prevention resources and tracks outcomes.

ESSENTIAL RESPONSIBILITIES

  • Initiates communication with patients upon learning of a positive pathology or finding, assuring a consistent point of contact for the patient in all aspects of cancer care. Guides patients through healthcare process from diagnosis to follow up, assisting in making appointments, registering patients through the system, facilitating interaction and communication with health care staff and providers, tracking interventions and outcomes. (50%)
  • Establishes relationships with key inpatient and outpatient sites to build referral mechanisms and promote Oncology Service Line. Identifies partners in oncology services in inpatient and outpatient centers involved in the diagnosis, treatment and care of cancer patients. Offers educational sessions to inform providers of the Oncology Navigator role, services provided, and referral mechanisms. (20%)
  • Connects patients to community and social support services, including American Cancer Society services. Assist patients and their families to overcome financial barriers to oncology services by identifying and securing payment options for care. (10%)
  • Provides ongoing evaluation and quality improvement of the Navigator program, developing relevant metrics and reports to measure and improve key processes and outcomes. Develops tracking mechanisms and plans for each patient to ensure that services have met their needs. (10%)
  • Attends and participates in Cancer Care Committee and Tumor Board. Works with cancer registry to ensure accurate information and patient statistics. (5%)
  • Coordinates activities to access for clinical trial and research program eligibility within WPAHS. (5%)

QUALIFICATIONS

Minimum

  • BSN required.
  • Pennsylvania RN license required.
  • Minimum of 5 years of Oncology Nursing experience required.
  • Demonstrated mastery-level understanding of oncology patient care and services.
  • Experience or ability to manage both inpatient and outpatient systems.
  • Familiarity with service area and oncology resources.

Preferred

  • MSN preferred
  • Certification in Oncology Nursing preferred.
