GENERAL OVERVIEW:

Provides care management, ongoing psychosocial assessment and treatment planning to patients within a variety of specialty areas in the medical setting per regulatory requirements. Collaborates with the multidisciplinary team, lending insight and expertise and is responsible to develop appropriate discharge plans and resources to enhance independence and community adjustment. Provides consultation in areas of expertise to hospital and social work staff.  Provides oversight of the AHNCI social work services program.  Provides operational oversight for the AHNCI social work services.  Responsible for ensuring program is exceeding customer, family and practitioner needs.  Serves as expert in proactively identifying and resolving issues.  Provides guidance and support to social workers.                      

ESSENTIAL RESPONSIBILITIES:

  • Completes and documents psychosocial assessments and provides social work services and disease management to patients, families and significant others in outpatient setting. (25%)
  • Assists in the development and implementation of discharge plan for specific patient populations. (25%)
  • Provides expert crisis intervention to patients and families. (15%)
  • Services as patient and family advocate to access community resources. (10%)
  • Assists in recruitment, retention, competency based orientation, onboarding and performance development for new hires. (10%)
  • Provides oversight and serves as expert in subject matter related to social services.  Responsible for onboarding, training, ongoing mentoring and competency assessment for social work team. (10%)
  • Other duties as assigned. (5%)

QUALIFICATIONS:

Minimum

  • Master’s degree in Social Work
  • Licensed Clinical Social Worker (LCSW) or licensure eligible in the state of Pennsylvania
  • 5-7 years social work experience is healthcare related setting
  • 1-3 years experience in senior social worker role
