GENERAL OVERVIEW

Provides an environment conducive to patients who may be at high risk for injury, falls or wandering due to various medical conditions. The primary responsibility is to ensure patient safety by providing continuous visual observation and appropriate intervention.

ESSENTIAL RESPONSIBILITIES

  • Provides for direct and continuous observation of patients under direct supervision of the registered nurse.  Carries out appropriate action taken to prevent or reduce the risk of harm to the patient such as limiting movement to avoid falls, preventing patient from removing intravenous lines and/or catheters, notifying staff if patient appears to have harmed self. 
  • Continuously monitors and immediately notifies the Registered Nurse caring for the patient of changes in the patient’s appearance and/or behavior including change in skin color, change in breathing pattern, change in responsiveness, agitation, combativeness or any behaviors that might represent increasing safety risk
  • Accompanies patient to any clinical tests or procedures off the unit.  Remains within sight of the patient unless otherwise directed by the person performing the test or procedure.
  • Assists RN in providing care in an emergency situation by alerting the RN to a change in the patient's condition, when necessary, initiating the code 99 process, anticipating needs and securing supplies as indicated.
  • Assists in keeping the patient’s room clean and orderly.
  • Maintains established policies and procedures, safety and environmental standards.

QUALIFICATIONS

Minimum

  • High school diploma/GED

Preferred

  • Prior sitter experience in acute care, extended care or home care preferred.
