GENERAL OVERVIEW:

Function as a clinician, consultant and educator in the delivery and evaluation of geriatric nursing care. Perform patient assessments and develop interdisciplinary plans. Provide nursing consulting services and implement nursing protocols for the Hospital Elder Life Program. Collaborate with interdisciplinary cadre of professionals and volunteers. Provide ongoing educational programs in geriatric nursing care. Mentor colleagues, including the Elder Life Specialist.                     

ESSENTIAL RESPONSIBILITIES:

  • Geriatric nursing assessment of all program patients, with implementation of nursing protocols for the program. (40%)
  • Consultant to nursing staff to provide comprehensive geriatric assessment and interventions with emphasis on prevention of cognitive and functional decline. (10%)
  • Coordinate interdisciplinary geriatric team rounds on each Hospital Elder Life Program nursing unit and assure clinical implementation of recommendations. (5%)
  • Monitor patients’ interventions and outcomes, including delirium risk factors and high risk medications. (15%)
  • Continually reevaluate program interventions and outcomes to respond to patient care needs and outcomes. (5%)
  • Serve as a clinical resource and as a role model regarding geriatric nursing care. (10%)
  • Provide nursing staff regular didactic educational sessions, case conferences and one-to-one bedside teaching. (15%)
  • Provide clinical and administrative support for Elder Life Specialist and volunteers.
  • Other duties as assigned or required.

QUALIFICATIONS:

Minimum

  • Bachelor’s degree in Nursing (BSN)
  • 3-5 years geriatric patient experience
  • Registered Nurse license

Preferred

  • Master’s degree in healthcare related field
  • 1-2 years teaching/learning experience
