GENERAL OVERVIEW:

Provides operational oversight for the AHNCI navigation program.  Responsible for ensuring program is exceeding customer, family and practitioner needs.  Serves as expert in proactively identifying and resolving issues.  Provides guidance and support to navigation team.                          

ESSENTIAL RESPONSIBILITIES:

  • Responsible for onboarding, training, ongoing mentoring and competency assessment for navigation team. (25%)
  • Proactively assesses and identifies opportunities for improvement of navigator program and role. Completes root cause analysis, educates and resolves issues in timely manner. (25%)
  • Works with manager in developing and updating navigation program policies and procedures. (20%)
  • Trends data and outcomes as established for the navigation program. Identifies gaps to improve patient care across the continuum. (20%)
  • Provides back-up navigation services determined by program needs due to attrition, leaves, etc. (5%)
  • Other duties as assigned. (5%)

QUALIFICATIONS:

Minimum

  • Associate’s degree in Nursing
  • 3-5 years experience in nursing experience in specialty field oncology
  • 1-3 years nurse navigation experience
  • Valid PA Registered Nurse License

Preferred

  • Bachelor’s degree in Nursing
