SummaryAs part of the growing need to transfer patients within AHN, Prehospital Care Operations is expanding the transport program to include ALS/BLS Ambulances to support community EMS when they are not available to provide the requested service. Responsible for providing care to those of chronic disease. The paramedic assists in disease management and transition of care to the home setting. The paramedic assists in education and understanding of disease processes, prescribed medications, and the use of medical devices that the patient will utilize in the home. Examples of understanding and use will include, recording of patient body weight, blood sugar, and blood pressure along with any other identified measurements to enhance healthy living.
